Sorry about the confusion; I tried to write things as clearly as possible. Each type of request/purchasing info had it's own "bullet", so the bullet for "Any customers who have already received a faulty GAMERA 3 replacement Blu-ray" specifically dealt with people like you who who have to make a second request to get the final Blu-ray. Info for first-timers was provided in the section starting with "If a customer has not yet sent their replacement request".
Hopefully that explains things a little better for anyone else here who needs to send in a first or second request.

No, it's my fault. I had actually second guessed this originally and asked someone else for their opinion and they were mistaken like I was, but it's still my fault at the end of the day. I'll send in a new request.

Mill Creek used brand-new high transfers, done by Kadokawa Pictures from the original film elements in the studio vaults.
Ah, my mistake. Should probably have looked that up.

My second replacement disc arrived in the mail and I finally had the time to view it. The subtitles on both the film and extra features are fine in terms of size. But they still seem a little buggy to me. Does anyone know just how accurate they really are?

Since BCI never got a chance to release
Silver Kamen, I highly doubt it unfortunately. I'd love to see the series that's pretty much Akio Jissoji's baby.
