You do have to send the cover art in the second time around. A rep from Mill Creek told me to do so.

kpa wrote:When Mill Creek sent me the info about the new G3 replacements they weren't specific one way or the other about the art wrap. But it was something I asked them about, and when Mill Creek did get back to me I added that info to the article and posted an update last week.
Any customers who have already received a faulty GAMERA 3 replacement Blu-ray can get the final corrected version by mailing a request to Mill Creek Entertainment. New requests must again include the artwork wrap as proof of purchase.

kpa wrote:Sorry about the confusion; I tried to write things as clearly as possible. Each type of request/purchasing info had it's own "bullet", so the bullet for "Any customers who have already received a faulty GAMERA 3 replacement Blu-ray" specifically dealt with people like you who who have to make a second request to get the final Blu-ray. Info for first-timers was provided in the section starting with "If a customer has not yet sent their replacement request".
Hopefully that explains things a little better for anyone else here who needs to send in a first or second request.

Mill Creek used brand-new high transfers, done by Kadokawa Pictures from the original film elements in the studio vaults.
